SURGICAL SOLUTION IN REPLANTATION OF THUMB AVULSION

SURGICAL SOLUTION IN REPLANTATION OF THUMB AVULSION (Abstract): This report presents a case of microsurgical replantation of the thumb with scarified some structure from adjacent digits. In a 42-years-old patient a cloth pulled by an electric rotating machine wrapping around the dominant thumb resulted a complete amputation at the base of the proximal phalanx. Examination revealed avulsion of both extensor and flexor tendons from their muscle bellies, both proper palmar digital nerves from median nerve and the princeps pollicis artery from the deep palmar arterial arch. Replantation was achieved successfully using artery, nerves and tendons from adjacent structures. A good functional and cosmetic result was accomplished. We conclude that the replantation of the thumb should be attempted in all cases even if we need to sacrifice other structures. In such cases, replantation is superior to any other alternatives to thumb reconstruction.
