PARTICULARITIES OF COMPLICATED INTRAABDOMINAL INFECTIONS ACCORDING TO ANATOMICAL LOCATION – A COHORT STUDY

      PARTICULARITIES OF COMPLICATED INTRA-ABDOMINAL INFECTIONS ACCORDING TO ANATOMICAL LOCATION – A COHORT STUDY (Abstract): Intraabdominal sepsis is a complex, expensive and frequenly lethal disease. Aim of the study was to define the incidence of intra-abdominal infections and the characteristics of the patients according to the location of the infection. Materials and methods: we performed a retrospective single centre cohort study of surgical patients admitted between January and December 2010 in University Emergency Hospital “Sf. Spiridon” an urban teaching hospital. Data were abstracted from computerized medical records. Results: Five hundred ninety-four consecutive patients with intra-abdominal sepsis were included in the study divided into three groups: single organ infections (IAS-IO), peritonitis (IAS-P) and intraabdominal abscesses (IAS-A). In studied cohort sepsis syndrome was present in 67.1% of patients, severe sepsis in 16% and septic shock and multiple organ failure in 16.9% of studied population. Median hospital and ICU LOS was 10 respectively 4 days and total mortality 14.5%. Peritonitis group had the highest severity of disease – 38.9% MSOF, the longest median ICU LOS- 6 days and the worst outcome – mortality of 32.2%. Conclusions: Complicated intraabdominal infections have an increase incidence among surgical patients. Mortality rates remain high, correlated with the anatomic location of infection, severity of the disease, presence of immunosuppression and type of causative microorganisms, the highest mortality being recorded in cases tertiary peritonitis with severe sepsis/septic shock and MSOF due to multiple drug resistant microorganisms.
